RN Hospice Case Manager is responsible for overseeing the care of patients who are nearing the end of life. The case manager works with an interdisciplinary team of medical professionals to ensure hospice patients receive high-quality palliative care, including physical, emotional, and spiritual support.
The hospice case manager also helps families navigate during this difficult time by providing care and support.
Hospice Case Manager Job Duties
Hospice case managers have a wide range of responsibilities including
- Meeting regularly with patients and their families to monitor the patient's physical, emotional, and spiritual needs
- Assessing patient's social and medical history to develop plans of care that meet their needs
- Working with physicians, nurses, and other medical professionals to coordinate care and treatment plans for each patient
- Maintaining records of patient care plans, physician orders, medication administration, and other information about a patient’s condition
- Coordinating care with other medical providers and social services agencies in order to meet patient needs
