POSITION TITLE: NURSE CASE MANAGER
DEPARTMENT: Case Management
REPORTS TO: RN, Case manager, Administrator
POSITION SUMMARY:
· Comply with standards of business conduct in accordance with federal, state and local health and regulatory standards and guidelines, as applicable.
· Must possess a valid LPN license issued by the State of New York
· Must be able to work well with peers in a congenial work environment, and above all, maintain the utmost compassion and dignity to the residents and adhere to the Resident Bill of Rights.
· In exigent circumstances, the employee may be subject to call back during emergency conditions (e.g., severe weather,
evacuation, temporary staffing shortages, etc.).
· Must adhere to Waterford on the Bay attendance policies and follow the terms and conditions of employment as set forth in the Employee Handbook.
· Must be able to read, write, speak and understand English.
· Must adhere to all facility policies and procedures.
· Additional duties as assigned
RESPONSIBILITIES/ACCOUNTABILITIES
· The Wellness Nurse is responsible to arrange for care, treatment, and medication to residents, and to continually monitor and assess the residents’ condition and report in accordance with plans of care, established medical practices, and all applicable laws and regulations, and facility policy and procedures.
· Assists with oversight of ancillary care staff
· Oversee Medication Department and administer meds in emergency situations.
· Supervision of Care staff
· Provides emergency assistance to residents with appropriate follow-up
· Obtains and transcribes physician orders
· Maintains case management documentation, ie. Admission, Re-Admission, Doctor Notes, Change of Condition
· Completes Resident Admission, Change of Care and Annual Medical Forms (3122, DSS4449C)
· Liaison to Family Members and Residents regarding their care, ie. Lab results, Med orders, Hospitalization
Annual Health REQUIREMENTS:
· Must provide physical at start of employment and annual plus verification of a negative TB skin test, QuantiFERON or chest xray, tb screen
Physical and Sensory Requirements:
· Must be able to move intermittently throughout the workday.
· Must be able to cope with the mental and emotional stress of the position.
· Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
· Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, agitated residents
· Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
· May be necessary to assist in the evacuation of residents during emergency situations.

Pay: $58,000.00 - $63,000.00 per year
