Complete Well-Care Source, LLC (CWCS) sets itself apart as a community based, non-profit organization that is dedicated to addressing the needs of those living with mental illness and promoting overall mental health for all eligible members. CWCS will fulfill its mission by providing services and resources that many of its members may not have been aware of otherwise. Such services include, but are not limited to, Peer Support, Supported Employment and Outpatient Therapy.
POSITION SUMMARY:
Under the supervision of the Director/Executive Director (or Designee), the LCMHC, LPC, LCAS, LCAS-A – Counselor will be a part of the primary care team, ultimately responsible for providing standardized counseling/talk therapy for Complete Well-Care Source, LLC (CWCS) clients with mild to moderate mental health and/or substance abuse conditions. The LCMHC, LPC LCAS, LCAS-A-Counselor will also be responsible for addressing additional social needs that clients may have by utilizing area referral sources for specialty, hospitalization, and/or advanced assessments needs. Work for this role is assigned in terms of, and in direct correlation with the functional/organizational objectives.
The Medical Director/Executive Director (or Designee) will provide guidance in unusual situations which do not have clear precedents and on an as-needed basis. In general, the Licensed Professional Clinical Counselor will (1) work independently to resolve problems based on past precedent; (2) exercise professional judgement in interpreting guidelines and applicability; and (3) ensure deadlines are met. In this role, the employee will also plan and carry out various stages of projects by selecting and pursuing approved methods and techniques as appropriate. Assignments are reviewed for quality and compliance with established policies and procedures.
POSITION RESPONSIBILITIES:
- Receives referrals from CWCS.
- Assess the mental and behavioral health status of referred clients.
- Provide counseling/talk therapy to scheduled clients utilizing professional counseling standards.
- Formulate differential diagnosis of mild to moderate mental health conditions and/or substance abuse.
- Formulate short-term and long-term counseling plans for clients under care.
- Identify clients with urgent mental health conditions (e.g., suicidal, homicidal, psychotic, etc.) and make appropriate referrals to available emergency or psychiatric treatment facilities.
- Provide written documentation of counseling sessions, assessments, interventions, and recommendations for CWCS providers and for the purpose of meeting regulatory requirements for medical records documentation.
- Provide non-scheduled urgent assessment of clients.
- Provide OPT(therapy) session as indicated.
- Assist Medical Director in developing protocols for CWCS counseling services and criteria for outside referrals for advanced care.
- Prepare monthly productivity reports.
- Prepare reports for quality improvement.
- Provide in-service training on relevant topics for co-workers as requested.
- Participate in a team management approach to patient care/treatment.
- Maintain an understanding of how behavioral health priorities are administered under State of North Carolina DHSS guidelines and consult with specialists and colleagues to review and participate in the care of clients.
- Maintain quality care through participation in peer reviews, policy development and quality improvement programs.
- Maintain appropriate documentation of clinical and counseling services, including all necessary billing information.
- Ensure accurate and consistent use of encounter forms for all behavioral health providers to ensure accurate billing.
- Maintain and grow professional knowledge and proficiency through continuing education, staff meetings, workshops and serving on special committees.

POSITION QUALIFICATIONS:
- Skills in providing professional counseling/talk therapy according to established standard of care; excellent customer service and support skills.
- Demonstrated ability to organize and prioritize workload and meeting deadlines; and excellent written and verbal communication skills.
- Demonstrated ability to successfully counsel clients for improved mental and behavioral health.
- Demonstrated ability to work with physicians, mid-levels and other members of medical care teams to evaluate patient conditions and develop comprehensive care plans.
- Demonstrated ability to successfully refer clients to mental health specialists and other community resources as appropriate.
- Demonstrated ability to interact effectively and professionally with persons from diverse cultural, socioeconomic, education, racial, ethnic and professional backgrounds.

MINIMUM QUALIFICATIONS:

- Must be computer literate with ability to enter information in the CWCS EHS system and compile reports or data as requested.

EDUCATION/EXPERIENCE REQUIREMENTS:
Completion of an accredited program with certification and licensure as a Licensed Professional Clinical Counselor in the State of North Carolina.
LICENSE/CERTIFICATION REQUIREMENTS:
- A Licensed Professional Clinical Counselor (LPCC) – A minimum of two years of full-time experience in outpatient mental health counseling, preferred
- Current CPR certification
- Valid North Carolina Driver’s License
- Must possess current North Carolina licensure as a Licensed Professional Clinical Counselor
- Must be able to meet and maintain current CWCS credentialing and privileging requirements
