Position Title: Bilingual (Spanish) Therapeutic Case Manager
Reporting to the Clinical Manager, the Therapeutic Case Manager will be responsible for providing case management services and clinical consultation within the assigned program at The Harbour. The Therapeutic Case Manager will work with members of the residential team and the youth directly, to deliver weekly case management, develop service plans, linkages to the community and supporting programmatic outcomes attainment.
Essential Functions:
- 1) Responsible for the delivery of therapeutic case management services to young adults:
o Develop and maintain positive relationships with youth in care and role model independent living and problem-solving skills.
o Maintain communication with collaborative agents involved with each client, i.e. legal advocates, therapist, probation officer, school social worker and visiting resources.
o Support the maintenance of a positive therapeutic milieu within each TLP site. o Meet with the youth weekly, minimally once a month in the home setting.
o Development and facilitation of clinical and/or psychoeducational groups for youth in our services.
- 2) Responsible for clinical assessment used to develop service plans, transition plans, and individualized crisis support plans for youth in the program.
o Partner with youth to develop, monitor and revise service, transition, and discharge plans within 30 days of admission and quarterly thereafter.
o Administer trafficking assessments at the Harbour’s emergency shelter within the defined number of days of the client's arrival to the program.
o Complete the initial mental health assessment within 45 days of admission into the program and update assessments annually.
o Develop with clients, a discharge plan to include provision of three-month aftercare services for program graduates. o Develop Individualized Crisis Support Plans upon admission and review every 6 months at a minimum.
o Provide weekly clinical and/or psychoeducational groups for youth in the program.
- 3) Responsible for documenting services offered, assisting with referrals, advocating for youth, and promoting positive outcomes:
o Process, review, and submit all unusual incidents and documents as required. o Complete case notes documenting all case contacts per DCFS’s Child Welfare License regulations and in accordance with grant guidelines.
o Maintain and update all client files per regulations under DCFS’s Child Welfare License regulations and in accordance with grant guidelines.
o Track outcomes for youth and participate in all CQI meetings.
- 4) Participate as a member of a multidisciplinary team to promote positive outcomes for youth:
o Develop and maintain positive working relationships with all team members. o Provide clinical input and feedback to Youth Development Specialists and members of the Residential Team.
o Offer consultation, guidance and support during business hours and after hours when on-call.
o Attend scheduled program, staff and agency meetings as required.
o Translate both written and verbal communication for program participants and staff.
o Provides clinical consultation and support to program staff members to ensure that therapeutic and trauma informed approaches are applied for all program participants.
Professional Development:
- Participate in scheduled supervision, bringing issues into supervision and demonstrating that material covered in supervision is utilized in job performance.
- Participate and secure at least 20 hours of in-service and/or external training annually (Minimum of 10 hours directly related to Human Trafficking), or CEUs required by your licensure, and demonstrate that material presented is integrated into practice.
- Participate in the Human Trafficking Task Force and other meetings relevant to Trafficking related services and community partners. Other duties as assigned.
Qualifications and Requirements:
- Master’s in Social Work, Bachelor’s in Social Work with 5 years' experience or equivalent.
- Bi-lingual Spanish/English Required
- Experience with SACWIS/ HMIS preferred
- Previous experience working with adolescents.
- Experience working with victims of human trafficking preferred.
- Experience working with victim of other traumatic crimes, such as domestic violence or sexual assault is required.
- Valid driver's license, auto insurance and a reliable vehicle for transportation of youth,
- Willingness to work flexible hours.
- Carry a cell phone and be available for rotating 24/7 on-call coverage for client emergencies.
- Obtain and maintain First Aid/CPR, and TCI certification.
- Operate office equipment: computer, fax machine, copy machine, smart phone.
- Able to walk up and down stairs, at least 10 times in an 8-hour shift.

Salary Range $46,000-$53,000
