We are new Home Healthcare company in Houston looking for a Registered Nurse to join our team. This person is responsible for performing in home visits for our Home Healthcare patients with a variety of medical conditions. He/she will administer medication, monitor and record patient progress, perform wound care, administer medications and educate families and the patient as needed.
The ideal candidate will display compassion, education, proficiency and experience while assisting the patient as well have have a history or familiarity with Home Health type visits.

Houston, Sugar Land, Missouri City, Stafford, Richmond area
Requirements:
Registered Nurse and a license to practice nursing in the state required
A minimum of six months of recent medical experience
Must have flexibility and reliable transportation.
Must be able to push, pull, reach, and bend frequently
Must be able to lift up to 50 pounds
Acute Care as well as Home Health experience preferred.
Responsibilities:
Provide services in accordance with the plan of care.
Makes the initial evaluation visit and regularly reevaluates the patients nursing needs.
Provides those services requiring substantial specialized nursing skills.
Initiates appropriate preventative and rehabilitative nursing skills.
Prepares clinical and progress notes for each patients visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
Informs personnel in changes in the conditions and needs of the patient.
Counsels the patient and the family/significant others in meeting nursing and related needs.
Participates in and presents in-service programs.
Understands and adheres to established Agency policies and procedures.
Process orders and notifies physician of patient needs and changes in condition.
Complete certification/re-certification orders and discharge summaries.
Determines the amount and type of nursing needed by each individual patient.
Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist, and Medical Social Worker those patients requiring their specialized skills
Supervises and teaches other nursing personnel.

Pay: $31.56 - $41.00 per hour
