Rutgers, The State University of New Jersey, is seeking an Advance Practice Nurse for NJMS- Division of Infectious Diseases. The Advanced Practice Nurse (APN) is responsible for providing primary care, managing health problems and coordinating health care for persons living with or at risk of HIV at the Infectious Disease Practice in accordance with State and Federal rules and regulations, nursing standards of care, clinic and DHHS guidelines. Responsibilities include, but are not limited to, taking medical histories, performing physical examinations, including women’s reproductive and sexual health care, ordering and interpreting lab tests, medications management, referrals to specialty care as medically appropriate and coordination of patient care with a diverse team of individuals.Consults with physician as needed. Additionally, the APN will be responsible for assisting clinic operations and being a key resource for nurses and ambulatory care technicians.
Among the key duties of this position are the following:
- Under the clinical supervision of the Medical Director, and guided by evidence-based protocols, and federal grant and state contract regulations, works with a multidisciplinary team to provide HIV prevention and treatmentservices, including medical assessment, diagnosis, treatment, referral and follow-up
- Formulates appropriate differential diagnosis, developstreatment plan and implements interventions to support the patient to regain or maintain physiologic stability. The plan is based on mutually established priorities to maximize the heath potential of the patient.
- Arranges for appropriate plan of care and follow-up based on outcome of diagnostic, lab, and physical assessment findings.
- Works collaboratively with the attending physician, consulting physician(s), and other disciplines to identify, develop, and implement an appropriate plan of care that maximizes individual patient/family preference and enhances quality, access, and cost-effective outcomes
