Position Summary:
The Supportive Living Coordinator is responsible for managing State Home Care and Supportive Housing program services for residents of a local Supportive Housing site, along with State Home Care consumers in the local community. This position provides linkage to needed support services for residents and works in partnership with the local housing authority (LHA) to advocate for resident needs and promote communal activities and onsite meal programs. This position is responsible for supporting the dignity and independence of persons with disabilities and older adults who need assistance by assessing their situations, identifying unmet needs, arranging for, coordinating, and monitoring services to meet those needs, and advocating with other agencies on their behalf.
Job Responsibilities and Performance Standards:
· Provide case management to oversee and monitor services and care coordination for older adults in the State Home Care Program, primarily those residing at a local Supportive Housing site. Conduct in-person assessments and resident safety checks via home visits.
· Determine eligibility for the State Home Care Program and/or other programs and resources in the community for Supportive Housing residents; makes referrals as necessary. Manage the provision of emergency and one-time-only services for housing residents as needed.
· Participate in and/or facilitates care conferences with residents, family members, the LHA, and community professionals regarding care needs and available resources and/or to mediate resident conflicts.
· Perform outreach and monthly communal activities for Supportive Housing residents to promote the availability of housing services, SCES’ programs, and social activities. Disseminate information to residents by creating newsletters, calendars, and/or informational flyers. Coordinate with other SCES staff on resident programming.
· Monitor resident participation in on-site nutrition programs and works with SCES’ Nutrition Department to ensure meals and programming fit the needs and wishes of residents. Assist residents with food insecurity through referrals to community programs, meals on wheels, and/or accessing SNAP benefits.
· Be knowledgeable about the LHA application process, move in and out procedures, and rules and regulations for the housing site. Assist building management as requested with service planning for residents who are relocating.
· Maintain and, if needed, create a Memorandum of Understanding with the LHA that ensures a positive working relationship and compliance with Executive Office of Elder Affairs (EOEA) Supportive Housing program requirements.
· Submit statistical and narrative reports to EOEA reflecting status of the Supportive Housing site and resident activities and engagement.
· Maintain documentation for consumer records and completion of required paperwork in an accurate, complete, confidential and timely manner. Does so in accordance with EOEA and Agency standards.
· Comply with HIPAA and other state and Agency confidentiality and documentation requirements.
· Weekly onsite office hours at assigned Supportive Housing site.
· Provide coverage for others as needed in the Home Care Department.
· Perform other duties as assigned including translation/interpretation assistance as needed if bilingual.

Qualifications:
· Bachelor’s degree required.
· Experience working within an interdisciplinary team preferred. Must be able to work independently and collaboratively with diverse populations and as part of a team.
· Strong leadership and group skills.
· Previous Aging Services Access Point (ASAP) and/or housing experience preferred.
· Exceptional time management and organizational proficiency. Should possess strong communication skills and the ability to multi-task in a fast-paced environment and ensure closed-loop communications.
· Must be knowledgeable and comfortable using Adobe and MS Word, Excel, SharePoint, and other Microsoft 365 platforms. Experience with Wellsky a plus.
· Must be accessible by phone, directly, or by coverage during normal business hours.
· Must meet both CORI/SORI and public health screening requirements
· Bilingual a plus.

Job Type: Full-time
Pay: $52,000.00 - $56,000.00 per year
