**Job Overview:**
We are seeking a dedicated LVN /Office Admin. to join our growing hospice team. The ideal candidate will have experience in hospice and possess a strong commitment to patient care. Office Skills are a must as well as most of the job duties will be in a office setting.
**Duties:**
- Becomes compliant with all departmental standards; policies and procedures, training and education, workforce management, disaster planning, effectively managing and prioritizing work tasks, attendance, adherence, and quality.
- Field work occasionally.
- Monitor patient vital signs and response to treatments
- Collaborate with interdisciplinary teams to provide comprehensive patient care
- Educate patients and their families on health management and disease prevention
- Maintain accurate medical records and documentation
-Promptly answers and screens incoming calls, with appropriate escalation as needed.
-Facilitates communication between team members, staff in the field, referral sources, vendors, physicians, facility staff, and patients and family.
- Assisting in HR and office duties.
**Requirements: **
- Valid LVN license in the state of practice
-Excellent interpersonal and communication skills, both written and verbal preferred
-Microsoft programs, EMR knowledge, HR experience, assist with onboarding, calls to patients / family/ providers.
- Previous experience in hospice
- Proficiency in patient assessment and diagnostic evaluation
- Familiarity with state and federal rules and regulations.
- Ability to work with diverse patient populations.
-Bilingial preferred but not required.
- Monitoring and completion of all reports needed for QAPI. Assist DCS with compliance of clinical staff as well.
- Strong communication skills and the ability to work effectively in a fast-paced environment
