Responsibilities:
- Conduct patient assessments and record medical histories.
- Administer medications and perform treatments as prescribed by physicians.
- Provide wound care and other nursing procedures as required.
- Educate patients and their families on disease management, treatment plans, and self-care techniques.
- Monitor patients' progress and communicate any changes to the healthcare team.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic care.
- Document patient care accurately and maintain confidentiality of patient information.
- Provide emotional support and counseling to patients and their families to promote well-being and coping strategies.
- Participate in interdisciplinary team meetings and contribute to the development of quality improvement initiatives.
Licensed Vocational Nurse Qualifications:
- Current Licensed Vocational Nurse (LVN) license in the state of CA.
- 1-2 years of clinical nursing experience, preferably in home health, hospice, or a related setting.
- BLS/CPR certification required.
- Valid CA Driver’s license.
