The duties of the Community Support Specialist include contributing to the development, implementation, monitoring, and updating of consumers’ individual habilitation plans; therapeutic rehabilitative skill development; implementation of Illness Management and Recovery (IMR) principles and interventions; coordinating and managing services, both mental health and physical health; crisis intervention, as needed. Provision/coordination of services aimed at active rehabilitation and support services to achieve restoration of functioning and the achievement of community integration; maintaining own appointment schedule and meeting work productivity requirements; other duties as assigned by management staff. Productivity requirement of one hour face to face contact per shift.
Community Support Services offers a variety of supports to help Burlington County adults with a serious mental illness remain living successfully in the community. Services include case management, housing assistance, supportive counseling, advocacy, service linkages, illness management, peer support, life skills, education/employment assistance, transportation and 24-hour on-call support.
Enhanced Community Support Services is also available for adults being released from state psychiatric hospitals. This program offers a higher level of support and case management.

Education/Experience:
Bachelor’s Degree from an accredited college or university in a helping profession, such as social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice. Bachelor’s Degree in a field other than a helping profession must have a minimum of two years of professional experience in a community behavioral healthcare setting. Certification as a psychiatric rehabilitation practitioner may be substituted for one year of experience.
