Job DescriptionSCOPE OF POSITION
The RN – Certified Clinical Nurse Specialist (CNS) functions within the following spheres of influence: patients & clients, nursing standards & nursing personnel, and systems. The Certified CNS facilitates and demonstrates expert nursing practice within his/her designated area of expertise. Essential functions of the Certified CNS role include:
A. The certified CNS is an advanced practice nursing professional who provides continuity and clinical coordination of care from admission to discharge. The certified CNS will have expertise in clinical decision making and the ability to suggest, interpret and test new concepts/standards in practice. The CNS demonstrates leadership by the promotion of caring practice, systems thinking and responsiveness to diversity by fostering clinical practice development through observation, conceptualization and analysis of complex clinical problems related to their area of expertise. The CNS provides guidance to staff nurses in routine and complex care and may serve as a direct care provider for patients with complex or special needs. The CNS collaborates with those necessary regarding clinical and administrative activities and in the design and implementation of cost effective patient programs related to their area of expertise.
B.
Direct Patient Care activities (if applicable, and credentialed) may include:
1.
Regular rounds on his/her patient population, maintaining and updating patient problem lists as needed.
2. May be the 1st call, in coordination with the provider team, for his or her inpatient population. May respond to emergency codes in area of expertise.
3. May order and evaluate appropriate diagnostic tests.,
4. Performing invasive and therapeutic procedures as approved by credentials committee, after appropriate supervision and competency testing, in collaboration with physician supervisor.
5. Communicating the established plan of care to the patient and family, Providing patient/family education
6. Facilitating communication between patient/families and the health care team.
7. Coordinating care plan with all appropriate members of the multi-disciplinary healthcare team to ensure continuous and efficient care.
8. Facilitating discharge planning with case management.
C. Guidance & Mentoring: The CNS serves as a role model and mentor to nursing staff relative to patient care concerns, evidence-based practice, resource identification, professional development, and research.
D. Education: Coordinates educational programs and presentations for nursing staff, patients & families, students, and area health care consumers. Assumes responsibility for personal professional development.
E. Consultation: Collaborates with all necessary members of the health care team as needed on clinical cases and issues in order to facilitate the provision of excellent patient care and achieve optimal patient outcomes. Facilitates evidence-based policy and procedure development, and works in an interdisciplinary manner toward system standardization. Represents designated area as the clinical and professional practice resource.
F. Research: Actively supports nursing research through data synthesis, research utilization, translation of research into practice, disseminating research findings, and/or generating new research investigations. Maintains collaborative relationships with area schools and colleges of nursing, and Ballad Research and Grants & Awards Departments. Participates in one or more Ballad Research Councils.
The Certified CNS is expected to practice autonomously within the scope of professional standards, the rules, and regulations established by the Tennessee Board of Nursing, including those for advanced nursing practice and is accountable for knowledge of applicable policies and procedures within Ballad Health.
The Certified CNS is an important member of the leadership team in his/her designated area of expertise. The role requires a commitment to healthcare quality, the ability to function autonomously, and excellent oral and written communication skills. The certified CNS identifies relevant patient care and/or nursing practice KIQ’s (key indicators of quality). This is achieved by using Plan, Do, Check, Act (PDCA) methodology and the appropriate data collection, aggregation, analysis and trending in the demonstration of measurable improvement outcomes.

EDUCATION EXPERIENCE
Currently licensed to practice professional nursing in the appropriate state. Graduate of an accredited school of nursing. Minimum of 5 years of progressive experience in area of expertise required. Masters degree in nursing with appropriate concentration, if available, in area of expertise. Currently possesses or is eligible for APN licensure in appropriate state, within 6 months of hire to position. Possession of, or ability to obtain prescriptive authority.
