Medical Assistant Experience
JOB SUMMARY:The Clinical Care Coordinator (CCC) provides hands on coordination of clinical care activities for Canton office. He or she serves as the clinical lead for care coordination and care management services for patients. Bilingual English/Spanish is strongly desired.
SPECIFIC TASKS AND RESPONSIBILITIES:
1. Actively participates in the continuous quality improvement of GHMS’ delivery of services.
2. Supports patient self-management of disease and behavior modification interventions.
3. Coordinates continuity of patient care with external healthcare organizations and facilities, including the process for hospital admission and discharge and referrals from the primary care provider to a specialty care provider.
4. Assists in reviewing charts prior to scheduled visits to assess patient needs in order to optimize the delivery of patient care at the visit.
5. Has working knowledge of clinic equipment and procedures.
6. Gives immunizations and other medications under supervision of physician and/or mid-level according to standing orders and protocols.
7. Helps maintain an inventory of supplies and equipment.
8. Will provide direct patient care as Medical Assistant when needed.
9. Works with front office staff and scheduling staff to ensure acutely ill patients are seen as needed.
10. Other duties as assigned.
QUALIFICATIONS
· Graduation from an accredited school of nursing or graduate of technical school in Medical Assisting or comparable experience in a medical office setting.
· 2-5 years’ experience in a clinical setting
· Experience with electronic medical records
· Proficient computer skills, including Microsoft Office and EMR (eClinical Works a plus)
· Self-disciplined, energetic, passionate, innovative
· A team player that can follow a system and protocol to achieve a common goal
· Highly organized and well-developed oral and written communication skills
· Demonstrates sound judgment, decision-making and problem-solving skills
· Able to maintain confidentiality with all aspects of information in accordance with practice, State and Federal regulations.
Bilingual in English/Spanish strongly desired.
