Our RN Care Manager is a unique position providing consulting and case management for Sisters of the Holy Names (SNJM) living in the East Bay area of California.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Advise individual Sisters with healthcare needs, including supporting Sisters in their independence and with their individual preferences.
- Performs essential functions of care coordination which may include assessment, development of health and related needs-based plan, implementation, monitoring, evaluation, and adjustment.
- Transports Sisters to medical appointments when needed and provides advocacy in the current healthcare environment, including accompaniment requested by a Sister for important appointments, during health transitions, for medical testing, surgery, etc.
- Transports/arranges transport/or meets Sister at the Emergency Department. Assists in care coordination and discharge planning.
- Serves as a resource to Sister and her chosen support person(s), especially during times of health transitions, serious medical conditions, and surgery.
- Assists Sisters in communicating with healthcare providers.
- Facilitates, coordinates, and monitors use of home care and other community- based services.

Education: Graduation from an accredited nursing program; BSN preferred.
Experience: RN with 5 – 8 years’ experience, including home health care management experience, or equivalent. Geriatric experience preferred.
Certification: Current RN license-State of California

- Thorough knowledge and understanding of the concepts of aging and concerns of the elderly population.
- Demonstrated ability to communicate professionally, empathetically, and compassionately with elder population who may be ill, disabled, hospitalized, and/or experiencing loss.
- Valid driver’s license and personal vehicle; good driving record.
- Ability to travel frequently within East Bay and Campbell areas to fulfill job responsibilities.
Position: Full-Time, 40 hours/week
Pay Range: $46.00 - $52.00/hour
