Home Instead is hiring for a full time Client Care Coordinator office position with a bonus program. Home Instead in Sherman Oaks, CA is seeking a relationship-builder and quality-driven individual to help us change the face of aging in our community. Candidates must be able to travel to Studio City, Sherman Oaks, and surrounding areas.
The Client Care Coordinator is responsible for a variety of duties that relate to client care, including care consultations with potential clients and family members, Client/CarePro ( Caregiver) introductions, and quality assurance visits with existing clients. A consultative sales approach is used to determine each individual client’s needs, to provide solutions, and to create a tailored service plan. This position continuously evaluates the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours.
Primary Responsibilities:
- Reflect the core values of Hummingbird LA, LLC d.b.a. an independently owned and operated Home Instead Franchise.
- Conduct client service inquires and care consultations as needed following the consultative sales approach.
- Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan.
- Maintain ongoing communication and a consistent follow-up schedule with potential clients and families who have yet to commit to service.
- Work with other team members to coordinate various aspects of a client’s care.
- Conduct client/Carepro introductions with every new client and with every new Carepro.
- Shared on call responsibility with key players
- Must be a good listener, be detail oriented, and able to prioritize effectively.
- Must be flexible and have the availability to work weekends, as necessary.
- Demonstrate open and effective communication with clients, family members, the franchise owner, colleagues and CarePros', and referral providers/care providers.
- Adhere to all company policies, procedures and business ethics codes and ensures that they are communicated and implemented within the team.
Minimum Qualifications:
- College degree preferred ( equivalent work experience may be considered).
- Licensed Vocational/Practical Nurse (LVN/LPN), Social Worker, Emergency Medical Technician (EMT), or Paramedic a plus.
- One year experience in home care, health care or senior-related industry required, or equivalent combination of education and work experience may be considered.
- Must possess a valid drivers' license
- Must have demonstrated computer skills and be proficient in Microsoft Office Suites with the ability to learn/use web-based software.
- Must be fluent in English, ability to communicate information effectively, possess a strong work and team ethic, ability to multitask with completing priorities independently and follow through to complete assigned tasks
