Principal Functions:
The Extended Hours Case Manager is responsible for the care coordination of services during the extended hours of services at SACHR. This includes assessment of new referrals, outreach to gain referrals, and coordinating medical, harm reduction, housing, and mental health services for participants. This position reports to the Co-Director of Programs.
Required Qualifications:
· At least one year of case management experience in a setting that serves individuals who actively use substances, or other vulnerable populations.
· Ability to work at least one of these shifts: Late evening availability (4pm - 12am); or weekend availability (8am - 6pm)
· Understanding of Harm Reduction Practices, Confidentiality, and HIPAA.
· Strong organizational, interpersonal, and written communication skills.
· Good computer & technical skills - proficient in word, excel and electronic record keeping.
· Ability to listen and respond non-judgmentally and with empathy.
Job Duties Include (but not limited to):
· Case management/Outreach/Advocacy as part of a multi-disciplinary team to active drug users at the central office and outreach locations.
· Engage with participants using a person-centered approach to help identify strengths/needs and establish goals and objectives.
· Recruit people in need of services who use drugs into program by understanding the unique challenges and oppression they face.
· Assist with referrals to community based services, housing applications and income/mainstream benefits applications, assistance in accessing health/mental health care, emergency food shelter, transportation and/or other social services.
· Complete documentation as required including electronic record keeping system, participant intakes, and completion of reports as required.
· Attend and participate in team meetings, case reviews and staff trainings as scheduled along with supervision and other program or agency meetings as assigned.
· HIV testing, pre/post HIV test counseling and referrals to STI/STD testing HIV/AIDS and STI/STD risk education and referral to appropriate care.
· Perform other duties as assigned.
Preferred/Special Skills:
· Spanish-speaking strongly preferred.
· Bachelor’s degree in a social science or related field strongly preferred.
· Outreach experience strongly preferred.
· Experience and knowledge of Harm Reduction as well as anti-racist and anti-oppressive frameworks. Experience with Trauma Informed Practice, Motivational Interviewing, Stages of Change or other evidence-based practices.
· Familiarity with Bronx mental health, medical, housing, and social service providers.

Pay: $50,000.00 - $60,000.00 per year
