Due to expansion and growth, we are immediately looking for four positions: two (2) Community Health Partners (CHP) and two (2) Peer Specialists professionals to provide education, case management support, and outreach across RaisingHealth's health, social service, and education programs, creating a streamlined care model across programs. The Peer Specialist/CHP help families understand and access community services and resources, while guiding them to adopt healthy behaviors.
The CHPs and Peer Specialists are REQUIRED to speak fluently in either Spanish & English OR Chinese & English. We will not consider candidates who do not meet this criteria. Two positions are needed for each language (Spanish (2) - Chinese (2). This is a federally funded grant position.
Responsibilities include but are not limited to:
- Providing culturally sensitive outreach and education on public health, social justice, immigrant and healthcare rights, and public benefits topics and for RaisingHealth Partners health and immigrant service programs
- Providing interpretation, follow-up, and case management for health and social services
- Enhancing civic engagement through awareness-building programming, presentations, and
- Developing and maintaining relationships with neighborhood stakeholders
- Building rapport with community members and provide ongoing follow-up, support, and advocacy.
Minimum Qualifications:
- High School Diploma required. Bachelor’s degree highly preferred.
- Prior case management, outreach, canvassing, or organizing experience preferred.
- Excellent communication skills, including the ability to listen well, speak articulately in a public setting and use language appropriately.
- Written and oral fluency in Spanish and English, Chinese and English required.
- Knowledge of some medical terminology preferred, but not required.
- Basic computer skills and knowledge in Microsoft Office.
- Ability to work a full time schedule - Tuesday through Saturday from 10am to 6pm
