Position Overview
The Illinois Department of Public Health is seeking a highly motivated individual to conduct surveys, investigations and monitoring visits of long-term care facilities to determine compliance with State Licensure requirements and/or Federal Medicare/Medicaid certification regulations. Prepares licensure and certification survey/investigation forms, memoranda and reports of findings. Conducts on-site monitoring visits at non-compliant facilities. This individual speaks and writes Polish at a colloquial skill level to members of the Polish speaking community.
Job Responsiblities
1. Surveys long term care facilities to determine compliance with state licensure requirements, federal certification mandates, and Department directives, program policies and procedures.
- Travels and conducts surveys to determine the compliance with state and federal licensure and regulatory requirements.
- Conducts revisit surveys to evaluate compliance with plans of correction.
- Observes patient care provided at facilities.
- Reviews medical records, policies, standard operating procedures, and other facility documents related to licensure and/or regulatory compliance.
- Interviews facility residents, staff, administrators, health care providers and other individuals.
- Conducts complaint and incident report investigations at regulated facilities or unlicensed facilities.
- Conducts entrance and exit conferences with various facility administrators and representatives.
- Collaborates with other surveyors on the survey team as either a team leader or survey team member, as assigned.
- Appears as a witness in Department or non-department hearings or court actions pursuant to documented survey findings.
2. Prepares written survey findings.
- Prepares survey/investigation forms and reports of findings as required.
- Documents survey/investigation findings in accordance with the mandated federal principles of documentation.
- Keeps supervisor advised of status and progress of survey documents.
- Submits survey package with all required documentation within the prescribed time limits.
3. Performs monitoring functions at noncompliant facilities.
- Conducts on-site monitoring visits at non-compliant facilities.
- Prepares monitoring reports and makes recommendations to supervisor regarding the status of the health and safety of resident(s) at monitored facilities.
- Conducts on-site investigations in response to incident reports or facility emergencies or disasters that impact the health and safety of facility residents.
- Keeps supervisor advised of status and progress of monitoring visits and emergency incident report investigations

4. Completes necessary surveyor training.
- Attends and completes basic surveyor courses as directed by a supervisor.
- Attends and completes trainings to maintain expertise in state and federal regulations and state licensure and certification requirements.
- Attends and completes mandatory federal trainings as designated by the Centers for Medicare and Medicaid.
5. Speaks and writes Polish to clients and groups. Answers inquiries from members of the Polish speaking community.
Provides technical assistance to facilities in emergencies.
- Provides assistance with relocation of residents in emergency situations.
- Ensures facilities comply with regulations for resident safety and resident rights during emergencies.
- Coordinates with other federal, state or local agencies assisting in the emergency.
6. Participates in staff meetings and assist with orientation of newly employed health facilities surveillance nurses and other Department staff as directed.
7. Performs other duties as required or assigned within the scope of the responsibilities enumerated above.
Minimum Qualifications
- Requires (a) graduation from an approved nursing education program resulting in an associate or a diploma degree in nursing and three years of professional nursing experience or, (b) a bachelor’s degree in nursing and two years of professional nursing experience or, (c) a master’s degree in nursing.
- Requires licensure as a Registered Nurse in the State of Illinois.
- Requires the ability to speak and write Polish at a colloquial skill level.
Preferred Qualifications
- Elementary knowledge in conducting surveys in skilled and intermediate care nursing home facilities and shelter care facilities.
- One year experience conducting survey activities in nursing home and related facilities.
- One year experience with federal and state rules and regulations concerning licensure and certification of long-term care facilities.
- Elementary knowledge of the on-site process of monitoring visits to non-compliant facilities.
- One year experience in nursing principles and practices.
- One year experience preparing accurate reports of survey findings.
- Ability to develop and maintain harmonious and productive working relationships with various administrative and medical personnel, local governmental agency personnel and recipients of health care program.
Conditions of Employment

- Requires a valid and current driver’s license.
- Requires ability to meet all agency vaccine/health-related polices and guidance.
- Must reside in Cook County. (May be assigned to facilities outside of that area, if necessary).
