Mission Statement
To provide individualized and compassionate care, education and support for the dying and those who have
experienced loss.

Job Summary
This position will be responsible for providing end-of-life care and hospice services to patients in their home or
community setting. The Licensed Practical Nurse (LPN) is responsible for assisting in planning for the patient’s and
family’s care under the direction of the RN-Patient Care Coordinator (PCC). The LPN will provide skilled nursing care
as directed by the RN-PCC and as described in the interdisciplinary plan of care, and will reinforce appropriate care
techniques. This position participates in the LPN weekend, holiday and on-call rotation.

Job Specifications

Education and Training – Graduate of an accredited LPN nursing program

License or Certification – Current LPN license by applicable state requirements; current BLS for Health Care

Providers or will obtain within 3 months of start date; valid driver’s license with acceptable driving record per
policy

Additional skills - Daily access to a reliable and insured vehicle. Must have strong written and verbal

communication skills, strong organizational skills, accurate and attentive to details, proficient keyboarding
skills, prior experience with electronic medical record systems and belief in the hospice concept.
Job Responsibilities

Provide nursing care, to include physical and psychological care, in accordance with established policies

and procedures of Seasons Hospice and consult with the RN-PCC regarding any changes in the care plan.

Maintain up to date patient records to ensure that all patient issues are accurately and clearly stated and

changes are reflected as they occur.

Knowledgeable of Medicare Hospice rules and regulations as well as Minnesota Standards of Practice

All charting related to patient care documented within 24 hours of visit and/or telephone communication.

Responsible to sync laptop daily to upload most recent patient documentation.

Responsible for maintenance of electronic medical records, assuring accuracy, completeness and

compliance with regulations, certification standards, legal and ethical standards.

Recognize and interpret symptoms and institute remedial measures within the scope/limits of a Licensed

Practical Nurse in the state of MN and report significant changes in the patient condition to the RN-PCC
immediately.

Administer medication and treatments as ordered by the physician, with the exception of IV push medication.

LPN must pass competency for intravenous/subcutaneous administration of medication via continuous
infusion. (LPN may increase or decrease dose, including delivering bolus dose via patient infusion pump, as
per instruction and delegation of duty by the RN-PCC.)

Reinforce appropriate interventions, medications, effects and side effects to patient, caregiver, volunteers

and other as appropriate as directed by the RN-PCC.

Responsible to ensure the patient has sufficient medications and supplies, per the care plan.

Participate in the interdisciplinary team (IDT) meeting to include exchange of information, review of

problems, and sharing of professional support.

Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure and

effective grieving for patient and family.

Identify and address comfort care needs.

Assist with patient cares at the Hospice House, as requested.

Support, instruct and educate the patient, family, and caregiver.

Coordinate patient and family services as directed by the RN-PCC.

Maintain the dignity of the dying patient.

Support the patients and family's unique spiritual and cultural beliefs.

Provide holistic, family-centered care across treatment settings to improve the quality of life.

Consult and collaborate with the interdisciplinary team and others involved in the patients care.

Provide care consistent with National Hospice and Palliative Care Organization standards of practice for

hospice programs.
Agency Expectations

Maintain absolute confidentiality of all information pertaining to patients, families, and staff. Adhere to all

Health Insurance Portability and Accountability Act (HIPAA) rules and regulations.

the community.

Serve as an ambassador for Seasons Hospice at all times.

Communicate clearly, concisely and accurately, creating a work environment that provides growth and job

satisfaction for self and others.

Participate in the employee performance review process.

Demonstrate ongoing responsibility for self education.

Maintain patient and personal safety regarding occupational health and safety.

Assume responsibility for the promotion of the hospice concept in the community and with other agencies

and professionals.

Actively participate in committees within the agency.

Attend scheduled department meetings and agency meetings on a regular basis.

Participate in the safety program and promote a safe work environment.

Participate in the Quality Assessment Performance Improvement Program (QAPI); as requested.

Participate in orientation, training and education of new employees and volunteers; as requested.

Participate in orientation and supervision for students seeking experience in hospice care; as requested.

Perform other duties as requested and/or assigned.
